You need to send the application to D&S DT, PO Box 418, Findlay, OH 45840. Send your payment and a copy of your training registration roster or challenge approval letter from the State.

4.How do I cancel my test?

You need to put your request to cancel in writing and send it to D&S so that it is received at least 1 business day prior to your test. If we receive it more than 7 business days prior to the test, you will receive a full refund if you were a self pay. If you were TDH funded, the facility will not be billed. If the test is less than 7 business days from when we receive your cancellation, you will be charged a $25 cancellation fee which will be deducted from your refund if you are a self pay.
